HEALTH OFFICE REMINDERS FOR 2010-2011

PHYSICAL EXAMS NEEDED FOR:

***Students in Pre-K or K, 2^nd, 4^th, 7^th and 10^th grades, any student NEW to our district and ALL students who are playing a sport are required to have a physical exam conducted by a physician.  The current physical form is on the back of this notice for your convenience.  If your child had an exam from Sept.09 to Sept. 10 it will be accepted.

DENTAL CERTIFICATE:

***** If your child has had a dental exam within the last year, please have your dentist provide a dental certificate (recommended not required).

Tdap VACCINE NEEDED FOR:

*****         Students in grades 6^th through 9th must show proof of a booster of the Tdap vaccine.*****

CHICKEN POX/VARICELLA VACCINE:

***** All students in K through 11^th grade are required to provide proof of immunity to the Chicken Pox disease.  This can be done by either documentation from your doctor that your child has had the disease or by having had the Varicella vaccine.

          If you have any questions please feel free to call your child’s school nurse and we will be happy to help you.
