WHAT MEDICAL DOCUMENTATION DOES YOUR
CHILD NEED TO START THE 2011-2012 SCHOOL YEAR?
Tdap Vaccine:
*****ALL students in 6th through 10th grade, they
must show proof of the Tdap vaccine.
VARICELLA
VACCINE/CHICKEN POX DISEASE:
*****ALL students Pre- K through 12th grade MUST provide proof of immunity to the
Chicken Pox disease. Your child needs to
have had the Varicella Vaccine OR documentation from your DOCTOR that they had Chicken Pox.
PHYSICAL
EXAMS:
***Students in Pre-K or K, 2nd, 4th,
7th and 10th grades and any student NEW TO OUR DISTRICT is required
to have a physical exam. If your child had an exam from September 2010 through
September 2011, it will be accepted for 11-12 school year.
STUDENTS
PLAYING A SPORT:
***ALL students
who are playing a sport are required
to have a current physical on file. This
physical must be done within the year prior to the start of their sport season. The physical form can be found on the school
web site.
DENTAL
CERTIFICATE:
***** Students in Pre-K, 2nd,
4th, 7th and 10th grades, if they have had a dental
exam within the last year, please have your dentist provide a dental
certificate (recommended not required).
