HEALTH OFFICE REMINDERS FOR 2010-2011

JV/Varsity fall sports begin on Monday, August 23^rd, and modified sports begin on Wednesday, September 8^th.

If your child is playing a fall sport, he/she will need to have a current physical exam on file in the health office. Physicals are valid for one year. If your child has a current exam on file, a “30 day update” form will be sent home to be filled out by you within 30 days prior to the start of the sport. These forms will be mailed, and need to be returned to the coach or the school nurse on the first day of practice. Physicals must also be on file in the health office by the start of the sport. Participation will not be allowed if these forms are not on file. Physicals can be faxed to (845) 373-4113.
